Applicants must be functionally independent. Assistance for scheduled care may be set up with AHS Homecare.
Application Procedure
Print an application form off of our website, or call the office to have one mailed to you. Notice of Assessment is required. Fill out both the application and have a doctor fill out the medical. Once all 3 (NOA, application and medical) are received, we will point score and call you to discuss your housing needs.
